Individual Course Approval

Please read the following information carefully!
If you do not pay the application fee, your submission is NOT considered complete. Paying the incorrect fee will delay the review process. Please see Section III for processing fees. FPTA does not accept checks for online submissions.
If you prefer another form of payment, please use this application form instead.
Section I – Individual Approval Overview
Individual Applications for continuing education approval should be submitted prior to the course dates or within (60) days of the course completion date. If the application is submitted more than 60 days from the completion date, the application is considered late and additional fees will be incurred (see Section III). Application submission does not guarantee approval.
If you are submitting a course for Individual Approval prior to taking the course, FPTA will not post your credit until we receive the Certificate of Completion. Therefore, you must email, fax (850) 224-5281 or mail the certificate back to the FPTA office upon completion of the course so that we may post your credit to CE Broker.
Once you receive your approval letter, it can take approximately 1-2 weeks to view your credit on the CE Broker website. All correspondence regarding your application, including your approval letter, will be sent electronically to the email address used on this form. Always check your spam folder if you have not received an email beyond the standard wait time (see below).
The entire approval process can take 4-6 weeks; however, it could take up to 8 weeks the closer it gets to the licensure renewal deadline. Therefore, it is highly recommended that licensees DO NOT wait until the end of the licensing period to apply for approval.
Section II – Required Attachments
Attachments are required for submitting a course for Individual Approval. A course brochure may suffice as long as it includes all the required information (see list below). If you were not given a brochure and/or it was given in a digital format, it is up to the individual seeking approval to gather the following information from the course provider:
1. CV or resume of speaker(s) clearly indicating credentials in area of course content (please note that bios included in brochures or advertising may not be sufficient). For large conferences with MANY speakers, short bios or a list of the speakers along with their credentials may suffice.
2. Program outline that must fully describe the time devoted to each topic.
3. Program objectives of the course.
4. Certificate of Completion indicating hours earned (this can be sent separate from your application if you are submitting prior to taking the course). See section I for more information about sending certificates.
Section III – Application Fees
$30.00 for courses worth 1-9 hours of continuing education and if submitted prior to the course dates or within sixty (60) days after attendance of the course. This fee is non-refundable whether or not the course is approved.
$50.00 for courses worth 10 or more hours of continuing education and if submitted prior to the course dates or within sixty (60) days after attendance of the course. This fee is non-refundable whether or not the course is approved.
$100.00 if submitted later than sixty (60) days after attendance of the course. If the course is not approved, the fee less $50.00 is refundable.
*Your email receipt will serve as confirmation that your application is complete. If you do not receive your electronic receipt for the application fee, please email info@fpta.org.
